You need Hammer Mastery 15 to equip it.

How to get it

The Sovereign Below gives it on turn-in if you hand it in holding a weapon of this style. It drops from The Bloated Sovereign (2.0%).

Break it down from your bag into 12–18 Venom Sac. While equipped it gives +28 Attack Damage and -5 Move Speed.
